Instructions for completing Form N ZP-Zdrav

1. By respondents in the form of federal statistical observation N ЗП-zdrav (hereinafter - the form) are all medical organizations, regardless of the departmental affiliation of the state and municipal forms of ownership (except for small businesses), carrying out economic activity in accordance with OKVED codes:

85.1 Health activities

Data on the average headcount and average headcount external part-timers(columns 1 and 2), as well as about the accrued fund wages(columns 3 and 5) on line 01 must be consistent with the corresponding data of form N P-4. For organizations with less than 15 employees that do not belong to small businesses and report in form N P-4 with a quarterly frequency, the data on line 01 must be consistent with the data of the corresponding columns of form P-4 for the corresponding period.

For other organizations reporting in form N P-4 with a monthly frequency, the data from the accrued wages fund on line 01 must be consistent with the sum of the corresponding data in form N P-4.

19. Row 01 reflects data on all personnel of the organization, p. 01 = p. 02 + p. 03 + p. 04 + p. 06 + p. 07 + p. 08 + p. 10 + p. 11 + p. 12 + p. 13.

Line 02 reflects data on the head of the organization.

Line 03 reflects data on deputy heads and managers structural units, who are not doctors (heads of accounting, personnel department, etc.), other heads of the organization (main nurse, chief midwife, chief paramedic, etc.).

1. Respondents in the form of federal statistical observation N ЗП-zdrav (hereinafter referred to as the form) are all medical organizations, regardless of the departmental affiliation of the state and municipal forms of ownership (except for small businesses), carrying out economic activities in accordance with OKVED codes:

2. If a legal entity has separate subdivisions, this form is filled out both for each separate subdivision and for legal entity without these separate divisions.

3. Medical organizations that include hospitals and polyclinics, including institutions conducting only consultative reception of patients, draw up a report for the entire organization as a whole.

4. The completed forms are provided by a legal entity to the territorial bodies of Rosstat at the location of the corresponding separate subdivision (for a separate subdivision) and at the location of the legal entity (without separate subdivisions). In the event that a legal entity (its separate subdivision) does not carry out activities at the place of its location, the form is provided at the place where they actually carry out their activities.

The XML-template of the form is posted on the official website of Rosstat gks.ru (Metadata and normative-reference information / Statistical reporting in electronic form / Schedule for placing XML-templates on the Internet portal of Rosstat) and on the official website of the territorial body of Rosstat.

5. The head of the legal entity appoints officials authorized to provide statistical information on behalf of the legal entity, including in its separate divisions.

6. The address part of the form indicates the full name of the reporting organization in accordance with constituent documents, registered in the prescribed manner, and then in brackets - a short name. On the form of the form containing information on a separate subdivision of a legal entity, the name of the separate subdivision and the legal entity to which it belongs is indicated.

On the line "Postal address" the name of the subject of the Russian Federation, the legal address with the postal code shall be indicated; if the actual address does not coincide with the legal address, then the actual mailing address is also indicated. For separate divisions that do not have a legal address, a postal address with a postal code is indicated.

The legal entity puts down the code in the code part of the form All-Russian classifier enterprises and organizations (OKPO) on the basis of the Notice of assignment of the OKPO code sent (issued) to organizations by the territorial bodies of Rosstat.

Geographically separate divisions of a legal entity, an identification number is indicated, which is established territorial body Rosstat at the location of the geographically separate subdivision.

The type code of the reporting organization (column 3 of the code part of the form) is affixed in accordance with the List of types of institutions in the social sphere and science for collecting and developing the results of federal statistical observation on the number and average wages of certain categories of social workers in relation to which measures to increase the average wages in accordance with the Decree of the President of the Russian Federation of May 7, 2012 N 597 "On measures for the implementation of state social policy", given in these Instructions.

7. Column 1 of the form provides data on the average number of employees payroll or the average number of employees for the reporting period, in column 2 - data on the average number of external part-time workers.

Average headcount employees for the period from the beginning of the year is determined by summing the average number of employees for all months that have elapsed since the beginning of the year, and dividing the amount received by the number of months for the period from the beginning of the year.

The average number of employees per month is calculated by summing up the payroll number of employees for each calendar day of the month, i.e. from 1 to 30 or 31 (for February - to 28 or 29), including holidays (non-working) and weekends, and dividing the amount received by the number calendar days months.

8. The calculation of the average number of employees is made on the basis of daily records of the payroll number of employees, which must be updated on the basis of orders for admission, transfer of employees to another job and termination of the employment contract. At the same time, for each day of the past period from the beginning of the year, it is necessary to distribute the payroll employees by categories of personnel, summarize the wages accrued for this period, calculate the average number of employees for each category, which will make it possible to reflect the transfer of the employee during the reporting period from one category to another (for example, if the employee worked for 6 months as a doctor, and for the next 6 months - as the head of a structural unit).

9. The number of employees on the payroll for a day off or a holiday (non-working) day is taken to be equal to the payroll number of employees for the previous working day. If there are two or more weekends or holidays (non-working) days in a row, the number of employees on the payroll for each of these days is taken to be equal to the number of employees on the payroll for the working day preceding the weekend and holidays (non-working) days.

The number of employees on the payroll for each day must correspond to the data of the time sheet of employees, on the basis of which the number of employees who showed up and did not show up for work is established.

10. The following employees are not included in the payroll:

a) hired part-time from other organizations;

b) those who performed work under civil law contracts (with the exception of employees who are on the staff of the organization and perform work under civil law contracts in the same organization);

C) transferred to work in another organization, if they do not retain their wages, as well as sent to work abroad;

D) directed by organizations for training in educational institutions with a break from work, receiving scholarships at the expense of these organizations; persons with whom an apprenticeship agreement has been concluded for professional education with payment during the period of the apprenticeship of the scholarship;

e) who submitted a letter of resignation and who stopped working before the expiration of the warning period or who stopped working without warning the administration. They are excluded from the payroll number of employees from the first day of absence from work;

E) military personnel in the performance of their military service duties.

11. When determining the average number of employees, the following should be taken into account.

A) The average headcount does not include following workers payroll:

Women on maternity leave, persons on leave in connection with the adoption of a newborn child directly from the maternity hospital, as well as on parental leave;

Employees studying in educational institutions and staying in additional leave without pay, as well as those entering educational institutions who were on leave without pay to pass entrance exams in accordance with the legislation of the Russian Federation.

b) Persons who worked incomplete work time in accordance with the employment contract, the staffing table or transferred with the written consent of the employee to work part-time, when determining the average number of employees, they are taken into account in proportion to the hours worked.

C) Employees who, in accordance with the legislation of the Russian Federation, have reduced working hours, including disabled persons, are accounted for as whole units in the average headcount.

d) Persons who worked part-time on the initiative of the employer are counted in the average number of employees as whole units.

e) The average number of employees in organizations that worked incomplete month(for example, in newly created organizations), is determined by dividing the sum of the number of employees on the payroll for all days of the organization's work in the reporting month, including weekends and holidays (non-working) days for the period of work, by the total number of calendar days in the reporting month.

Newly created organizations do not include organizations created on the basis of liquidated (reorganized) legal entities, separate or non-independent divisions.

12. The average number of external part-time workers is calculated in accordance with the procedure for determining the average number of persons who worked part-time.

The average number of external part-time workers for the period from the beginning of the year and for the year is determined by summing the average number for all months that have passed since the beginning of the year and dividing the amount received by the number of months in the reporting period.

13. Columns from 3 to 5 include on an accrual basis the sums of remuneration of payroll and external part-time employees accrued for the reporting period.

14. The accrued wages fund in columns 3 - 5 includes all amounts of payments, regardless of their funding sources, budget items and tax benefits provided.

According to columns 6 - 11 - the accrued wages fund of all employees (from column 3) and the accrued wages fund of external part-time workers (from column 5) are distributed according to funding sources: columns 6 and 9, respectively, reflect data on funds received from the budgets of all levels, in columns 7 and 10 - information about the means of compulsory medical insurance (compulsory medical insurance), in columns 8 and 11 - funds from income-generating activities and other funds.

15. An employee who receives in one organization two, one and a half or less than one rate, or is registered in one organization as an internal part-time worker, is counted as one person (a whole unit). At the same time, an employee who is on the payroll of an organization and performs work on an internal part-time job is recorded once at the place of his main job, the wage bill in column 3 shows the amount of wages, taking into account the part-time pay; in column 4 of column 3, the amount accrued to the employee for work on the basis of internal part-time employment is highlighted.

If an employee in the main job belongs to one category, and in internal part-time jobs - to another than the main job, then he should be reflected once - in the category to which his main job belongs. At the same time, column 3 on this line reflects the total earnings - for the main job and for the internal part-time job, and in column 4 from column 3, the amount accrued to the employee for work on the basis of internal part-time job is highlighted.

For example, if a paramedic internally works as a medical orderly, then he should be reflected as a unit on line 10 in column 1, and in column 3 his total salary as a paramedic and as a medical orderly should be reflected, in column 4 - only internal part-time salary , i.e. wages of the orderly.

16. An employee who is on the payroll of an organization and who has entered into a civil law agreement with the same organization is counted once at the place of his main work, and the wages accrued to him employment contract and a civil contract - the total amount in the payroll of employees on the payroll (in column 3); in column 4, remuneration for work under a civil contract is allocated. Within the framework of this statistical observation, work under a civil law agreement concluded by an employee on the payroll with his organization is considered as internal part-time job.

17. Columns 1 and 2 do not include persons who work only under a civil law contract and are not included in the payroll and the number of external part-time workers, and column 5, respectively, does not reflect the amount of remuneration to persons working only under civil contracts. Column 5 also does not take into account the amount of remuneration under civil contracts paid by this organization to persons who are external part-time workers.

18. More detailed methodological instructions for filling out data on the average headcount and the accrued wages fund are given in the "Instructions for filling out forms of federal statistical observation NN P-1, P-2, P-3, P-4, P-5 (m) ", approved by order of Rosstat dated 24.10.2011 N 435.

Data on the average headcount and the average number of external part-time workers (columns 1 and 2), as well as on the accrued wages fund (columns 3 and 5) on line 01 must be consistent with the corresponding data of form N P-4. For organizations with less than 15 employees that do not belong to small businesses and report in form N P-4 with a quarterly frequency, the data on line 01 must be consistent with the data of the corresponding columns of form P-4 for the corresponding period.

2. A clinic can be created as a structural unit of an educational or scientific organization carrying out medical activities.

3. Certain types of medical organizations specified in clauses 1.3, 1.5, 1.12, 2.1, 2.3, may be created in the form of structural units of medical organizations and other organizations that carry out medical activities along with the main (statutory) activities.

4. Specialized orphanages include orphanages for children:

with organic damage to the central nervous system with mental disorders;

5. The center of medical and social rehabilitation includes:

6. The list of types of institutions in terms of healthcare institutions is given in accordance with the nomenclature of medical organizations approved by order of the Ministry of Health of Russia dated 09.24.2012 N 211n.

If the institution carries out activities in the field of health care, then in the payroll settings, you must enable the function "Enterprise of the social sphere".

If the staffing table is maintained, then for each position you need to fill in the item "Position category for ZP-Zdrav". To do this, open each position staffing table and on the "Additional" tab, fill in the "Position category" item in the same way as indicated in the position for this position. In our case, the position of "Chief Economist" is filled as other personnel, respectively, we also choose here.

Also, all data on the number of employees can be checked using the report "Average headcount", this report is located on the tab "Personnel" - "HR reports" - "Average headcount".

After we have filled in all the necessary data, we can generate the "ZP Zdrav" report, for this we go to "Reporting" - "1C - Reporting", click "Create", select the report type "Statistics" - "ZP - Zdrav".

After the report opens, click fill and see the result. Also, by clicking the "Send" button, we can send a report to statistics if the 1C - Reporting service is connected.

  1. The proposed template corresponds to the reporting form No. ЗП-zdrav "Information on the number and remuneration of health care workers by categories of personnel", approved by order of Rosstat No. 574 dated October 30, 2012.

  2. Template name
The name of the template containing the report can be arbitrary, however, the length of the name should not exceed 20 characters.

You need to start filling out the template from the sheet "General information".

You need to fill out the sheet in the following order:
