Employee bonus order can be drawn up by filling in:

  • unified forms T-11 and T-11a, approved by the decree of the State Statistics Committee of the Russian Federation dated 05.01.2004 No. 1 (hereinafter referred to as the decree No. 1);
  • a free-form form developed and approved independently by a specific organization.

In both cases, the order will have legal force, since from 01.10.2013 the need to use exclusively unified forms for its execution has been canceled (see information of the Ministry of Finance of Russia dated 04.12.2012 No. PZ-10/2012, section "Forms of primary accounting documents" ).

In this case, in any case, the order form must include the information necessary for this kind of documents (see clause 2 of article 9 of the Federal Law "On Accounting" dated 06.12.2011 No. 402-FZ):

  3. Name of the organization.

  • To draw up the document, the official form of the enterprise is used, on which all its details are indicated (for individual entrepreneur Is the code of his enterprise in All-Russian classifier enterprises and organizations). The date of filling is stamped on it and official number order.
  • The order indicates the name, surname and patronymic of the employee (in accordance with the passport), the name of the structural unit in which he works, and the position he holds, as well as his personnel number, if any.

  • The line "type of incentive" indicates the "bonus" and the amount that will be issued, first in numbers, then in words.
  • In the line "grounds for encouragement" the name of the document, on the basis of which the employee is awarded the bonus, is indicated. Usually this is a petition of the head of a structural unit or the director of the enterprise himself, a description of the employee with an indication of his work experience or personal qualities or a report on his outstanding work success, which simply needs to be encouraged.
  • When the order is filled general director of the company indicates its own name, surname and patronymic, the position occupied by it, signs, decrypts the signature and puts the seal of the organization on the document.

The differences between the T-11a form and the T-11 form, the filling of which is described above, are minimal.
